Intervertebral bursitis confirmed with clinically suspicious arthralgia

(HealthDay)-Approximately one-quarter of patients with clinically suspicious joint pain (CSA) suffer from metatarsal interosseous bursitis (IMB), often associated with asymptomatic synovitis and tendonitis. increase. Rheumatoid arthritis..

Bastiaan T from Leiden University Medical Center in the Netherlands. van Dijk et al. Conducted a large magnetic resonance imaging (MRI) study to assess the development and prognostic value of IMB in 577 CSA patients. Participants underwent contrast-enhanced MRI of the forefoot, metacarpophalangeal joints, and wrists. MRI was evaluated for asymptomatic synovitis / tenosynovitis / osteoitis and IMB. We evaluated the association between IMB and asymptomatic inflammation detected by other MRIs.

Researchers found that 23 percent of participants had an IMB in a presentation with CSA. IMB was more frequent in anti-citrullinated protein antibody (ACPA) -positive and ACPA-negative CSA (47% vs. 19%). The likelihood of asymptomatic synovitis and tenosynovitis was increased in patients with IMB (odds ratios, 3.4 and 5.9, respectively). The risk of developing arthritis increased with IMB (hazard ratio, 1.6) after adjusting for other asymptomatic inflammation. The presence of IMB predicted the development of arthritis in CSA patients who were ACPA positive but not ACPA negative (adjusted hazard ratio, 2.2).

“IMB is detectable by MRI in a quarter of CSA patients and is often associated with asymptomatic synovitis and tendonitis. In addition, IMB precedes the development of clinical arthritis, especially in ACPA-positive CSA.” The authors write. “These results support the idea that synovial inflammation near the joints is involved in the early stages of the onset of arthritis in rheumatoid arthritis. arthritis.. ”
